Tous incidents/accidents sur le site du CERN ou sur le trajet CERN/domicile doivent être déclarés sur EDH

Les procédures applicables en cas d'accident et accidents évités de justesse varient selon le type d'évènement.

Pour les accidents non-professionnels :

Assurance maladie | Régime d’assurance maladie du CERN (chis.cern)

En cas d’accident non-professionnel, en plus du formulaire CHIS/F01, il faut joindre le formulaire complété CHIS/F02-Déclaration d’accident non professionnel. Par la suite, pour les demandes de remboursement pour le même accident, il suffira de cocher la case “Accident” sur le formulaire CHIS/F01 et d’indiquer la date de l’accident.

For non-occupational accidents:

Health Insurance | CERN Health Insurance Scheme (chis.cern)

In the event of a non-professional accident, in addition to the CHIS/F01 form, the completed CHIS/F02- Declaration of non-occupational accident form must be attached. For subsequent claims for reimbursement for the same accident, simply tick the accident box on the reimbursement form and indicate the date of the accident.
